Barnardo's work in partnership with Lincolnshire County Council to deliver its Leaving Care Service for young people aged 16-25 years old. We support, encourage, and empower care experienced young people to live independent lives.

Due to the service expanding this has created two new Leaving Care Worker positions, one position based in Lincoln, supporting care experienced young people aged 16 - 21 years old and a further position based in Sleaford to support 21-25-year-old care experience young people, both roles are exciting positions and new additional posts within the service. The 21- 25-year position will include developing a step forward service for post 21 care experienced young people.

Initial Specific Responsibilities
Within Lincolnshire the Signs of Safety model is used across County as an assessment tool. Offering information, advice, guidance and support with Accommodation, Health, EET, Finances and Independent skills.

• Carry out Pathway Plans Reviews which include recognising assessing and managing risks in the form of danger statements and safety planning, carrying out statutory face to face visits travelling nationally to deliver these and writing comprehensive supervision overviews of case management.
• Actively work with post 21 years old care experienced young people to complete a time-framed focused support with Care experienced young people.
