A bit about the role
Action for Children Fostering Scotland Service is a specialist fostering service that offers attachment-based therapeutic care to children and young people who cannot be looked after at home.  We offer the highest standard of care, providing carers with excellent training, supervision and support to achieve the best outcomes for the young people we look after.  We have had an excellent Fostering Inspection and continue to ensure excellent quality of care to support young people. We listen to young people every step of the way.

As a Transitions Worker you'll be providing one to one (and possibly group based) practical and emotional support to 15-25 year old young people to make the transition from foster care/continuing care into independent living. You'll coordinate and direct the support to young people by agreeing and providing interventions, enabling positive and agreed outcomes to be achieved, in accordance with organisational policies, procedures and regulatory requirements.

The contract initially runs for 18 months – with the potential of extension. It will enable you to grow your experience and build your network with a wide range of organisations across the region and gain invaluable experience within a great Charity.   

How you'll help to create brighter futures
As a Transition Worker you'll be part of a team that values all of our relationships and where relational practice is at the heart of all that we do!

Some key responsibilities will be;

• Bringing your experience, values and enthusiasm to a team (social workers, young person's worker and consultant therapists) supporting fostering families to provide stable and loving homes for young people.
• You will be involved in delivering the Fostering Scotland Team's participation strategy.
• Working with others to inspire, support and keep young people safe, to create good memories and support young people's connections with their families.
• Promoting the highest standards of care and wellbeing for our young people by really getting to know them and supporting them through their journey.
• By thinking therapeutically and creatively you will make a real and lasting difference.

Let's talk about you:

• SVQ3 or above and a willingness to undertake further training.
• Previous experience of working with young people who have experienced significant loss and trauma.
• Experience in life story work would be desirable.
• The ability to use a variety of tools and approaches to support young people to express their feelings and views.
• Experience in group work with young people, in areas such as confidence building and sexual health.
• Driving licence and access to a car. The job requires the ability to travel according to the needs of the job, with reasonable adjustments, if required, according to the Equality Act.
